Why You're Always Tired — And Why Your Doctor Hasn't Found the Answer Yet

Why You're Always Tired — And Why Your Doctor Hasn't Found the Answer Yet

You've tried sleeping more. You've cut back on caffeine, then added it back. You've taken B12 supplements from the drugstore. You've had your thyroid checked — twice. And every time your doctor runs bloodwork, the answer is the same: "Everything looks normal."

But you don't feel normal. You feel exhausted in a way that sleep doesn't fix. You push through your days running on willpower and coffee, wondering why everyone else seems to manage just fine while you're quietly struggling to keep up.

If this sounds like your life, I want you to know two things.

First: you are not imagining it. Second: "normal" lab results don't mean nothing is wrong. They mean the right questions haven't been asked yet.

In my practice, persistent fatigue that doesn't respond to rest — especially when conventional testing has come back unremarkable — is one of the most important signals I take seriously. And more often than not, the answers are there. They're just not where standard medicine looks for them.

Why Standard Labs Miss What's Really Going On

Conventional bloodwork is designed with one primary goal: to rule out serious disease. It does that job well. What it is not designed to do is identify the subtle, cellular-level dysfunctions that quietly drain your energy long before anything registers as "abnormal" on a standard panel.

Here's what most fatigue patients have never been tested for:

  • Mitochondrial dysfunction — your mitochondria are the energy factories inside every cell in your body. When they're not working efficiently, fatigue is the inevitable result. Standard labs don't assess mitochondrial function.

  • Functional nutrient deficiencies — you can have blood levels of B12 or folate that fall within the "normal" reference range and still not have enough for your cells to use efficiently. Standard labs measure presence, not function.

  • Gut dysbiosis producing systemic fatigue — imbalanced bacteria and yeast in the digestive tract produce metabolic byproducts that circulate throughout the body and directly impair energy production and brain function.

  • Neurotransmitter pathway dysfunction — the biochemical pathways that produce serotonin, dopamine, and other brain chemicals depend on specific nutrients. When those pathways are disrupted, fatigue and mood changes follow.

  • CoQ10 depletion — CoQ10 is essential for mitochondrial energy production and is one of the most commonly depleted nutrients in chronically fatigued patients, particularly those on statin medications. It is almost never tested in conventional medicine.

  • Amino acid and fatty acid imbalances — the raw materials your body needs for cellular repair, hormone production, and brain function. Deficiencies here are silent but far-reaching.

None of these show up on a standard metabolic panel or CBC. But all of them can be measured — precisely and comprehensively — with the right functional test.

The Tests That Actually Answer the Question

Two of the most powerful tools I use in my practice for uncovering the root causes of fatigue are Metabolomix+ and NutrEval FMV, both offered through Genova Diagnostics — one of the most respected functional medicine laboratories in the country.

Both tests are built around organic acid testing — the analysis of small metabolic byproducts excreted in urine that give us a detailed window into what's happening at the cellular level. Think of it as reading the exhaust from an engine: you can't always see what's wrong by looking at the outside, but analyze what's coming out and the picture becomes very clear, very quickly.

Here's how the two tests compare and who each one is best suited for.

Metabolomix+ — A Comprehensive Starting Point

Metabolomix+ is Genova's comprehensive urine-based nutritional and metabolic panel. The base test is collected from a first-morning urine sample at home — no clinic visit required for collection — and covers:

  • Full organic acids panel including mitochondrial energy markers, B-vitamin functional status, neurotransmitter metabolites, detoxification indicators, and gut dysbiosis markers

  • Oxidative stress markers assessing free radical damage and antioxidant capacity

  • Intestinal microbial overgrowth indicators including yeast, fungal, and bacterial dysbiosis markers

For patients who want to go deeper, Metabolomix+ can be expanded with optional blood spot add-ons collected via a simple finger prick at home:

  • Essential and metabolic fatty acids

  • Amino acid analysis

  • Nutrient and toxic elements, including mineral status and heavy metal screening

Metabolomix+ is the right starting point if you:

  • Are newer to functional testing and want a meaningful but accessible entry point

  • Prefer to avoid blood draws

  • Have symptoms centered primarily around fatigue, gut health, and energy

  • Want to begin with a focused panel and expand based on findings

NutrEval FMV — The Most Comprehensive Single Test Available

NutrEval FMV (First Morning Void) is Genova's most thorough nutritional and metabolic assessment — and in my opinion, one of the most comprehensive single tests available in functional medicine for patients with complex, longstanding fatigue.

It combines a first-morning urine collection with a blood sample (finger-prick blood spot at home or a standard blood draw) and delivers a fully integrated picture of:

  • Full organic acids panel — all energy, B-vitamin, neurotransmitter, detoxification, and dysbiosis markers

  • Amino acid analysis — built into the core panel, not an add-on

  • Fatty acid analysis — omega-3 and omega-6 ratios, inflammatory fatty acid markers

  • CoQ10 levels — critical for mitochondrial energy production and rarely tested elsewhere

  • Oxidative stress markers

  • Nutrient and toxic elements — essential minerals and heavy metal screening

NutrEval also generates a Metabolic Needs Assessment — a visual, clinician-friendly summary that translates the biochemistry into clear priorities and targeted supplement recommendations based on your specific results.

NutrEval FMV is the stronger choice if you:

  • Have complex, multi-system symptoms — fatigue alongside brain fog, mood changes, gut issues, poor sleep, or hormonal disruption

  • Have been struggling for a long time without answers

  • Want the most complete nutritional baseline available from a single test

  • Are ready to invest in a thorough assessment as the foundation of a comprehensive treatment plan

Metabolomix+ vs. NutrEval — At a Glance

For most of my patients presenting with chronic, unexplained fatigue — especially those who have been struggling for years — I lean toward NutrEval because it gives us the most complete picture in one integrated report. For patients who are newer to functional testing or have a more focused presentation, Metabolomix+ is an excellent and meaningful first step.

The right choice depends on your symptoms, your history, and what we're most trying to understand. That conversation is exactly what an initial consultation is designed for.

What We Find — And What Changes

I want to share some of the most common patterns I see when these tests come back, because understanding what we're looking for makes the value of this testing concrete.

Mitochondrial dysfunction is present in the majority of chronically fatigued patients I test. Specific organic acid markers tell us exactly where in the energy production cycle the breakdown is occurring — and that tells us precisely what to support.

CoQ10 depletion is remarkably common, particularly in patients over 40 and in anyone who has been on a statin medication. CoQ10 is essential for the final step of cellular energy production. When it's depleted, fatigue is one of the first and most prominent symptoms — and it won't improve without targeted replenishment.

Gut dysbiosis markers frequently emerge in fatigued patients who don't consider themselves to have significant digestive issues. The connection between gut imbalance and systemic fatigue is well-established in the research — but it's easy to miss if you're not looking for it.

B-vitamin functional deficiencies — particularly B12, B6, and folate — show up regularly even in patients who supplement, because the issue is often not intake but utilization. This is especially relevant in patients with MTHFR gene variants, which affect how the body processes folate.

Neurotransmitter precursor imbalances explain the fatigue-plus-mood picture that so many patients describe: not just tired, but flat, unmotivated, anxious, or emotionally depleted alongside the physical exhaustion.

When we identify these patterns and address them directly, the changes patients experience are often significant. Not overnight — genuine healing takes time. But the trajectory shifts in a way that symptom management alone rarely achieves.

Is This Testing Right for You?

Consider a comprehensive functional nutritional assessment if you're experiencing:

  • Fatigue that doesn't improve with sleep or rest

  • Brain fog, poor concentration, or memory concerns

  • Mood changes — anxiety, depression, or emotional flatness

  • Chronic digestive complaints

  • Poor sleep quality despite feeling exhausted

  • Slow recovery from exercise or physical activity

  • A history of long-term medication use that depletes nutrients (statins, metformin, proton pump inhibitors, oral contraceptives)

  • Chronic antibiotic use

  • Normal conventional labs despite persistent symptoms

  • A desire to understand your nutritional and metabolic baseline before beginning any supplement or treatment protocol

If you've been told everything looks fine but you know something isn't, this is often where the real answers begin.

One More Thing

These tests are tools. Powerful ones — but tools nonetheless. What makes them valuable is having a provider who knows how to interpret the data in the context of your full clinical picture and translate it into a targeted, individualized plan.

Ordering a NutrEval or Metabolomix+ without that clinical context is like receiving a detailed map with no idea where you're trying to go. The information is rich. What matters is knowing what to do with it.

That's exactly what I'm here for.

If you're ready to stop guessing and start understanding what's actually behind your fatigue, I'd love to help. Schedule a consultation and let's look at the full picture together.
